Requirements for Within District 3
transfers
- The Within District
Declaration of Residence (WDR) must be
completed by the player first.
- The completed documents
and forms must be submitted to the receiving association Registrar ASAP
- Upon receipt the Receiving
Association Registrar must verify that all documentation is accurate and
complete. Then gets the 2 signatures from the receiving association needed
at the bottom to complete the form
- The receiving registrar
must then fill out a Member of Good Standing (MofGS)
form with the player contact information and forward it to the District
Registrar with the Within District 3 Declaration of Residence and proof
(drivers license, utility bill, agreement of purchase, lease etc..). The District Registrar will then send
the 2 forms to the sending association.
- The MofGS form and the
WDR, when completed by the sending association ,should include any and all
information in the comments field which indicates the parent’s and
players status with the
association, prior suspensions and discipline actions. The sending
association forwards everything to the District 3 Registrar.
Residency Requirement
o Residential
purchase document (proves purchase)
§ Hydro Bill
or Gas bill (proves occupancy)
o 1 year
lease agreement, must cover from August 1 to July 31st
o Cannot be a
sub-let, must be sole resident
o Lease
residence must be able to contain Parent and child (two rooms)
o If leasing
a residence from a family member a sworn affidavit indicating they are the sole
occupants of the residence must be provided and that they will reside at that
residence from August 1st to
July 31st.
Separation Requirements
o Legal
separation documents must be provided with court sanctioned custody.
o Players
must provide proof that they are registered in the school district where they
intend to play.
o The
declaration of minor hockey association cannot be changed unless the custody
status changes
o Appeals - All appeals are to the District where the
either the IDR or MofGS application was rejected and with all involved parties
present.
